getCaptureCublasHandle serves every capturing stream of a thread with one rocBLAS handle, but it copied the pointer mode from the stream's eager handle only the first time a (capture, handle, stream) was seen. A caller that set device pointer mode on stream A, then used stream B (host mode) in the same capture, got B's host mode back on A, so device-pointer scalars were read as host pointers.
